Key Responsibilities\n

• Conduct psychosocial assessments of patients and their families to identify emotional, social, and environmental strengths and challenges related to their diagnosis or life circumstances, providing a foundation for tailored intervention plans.

• Develop, implement, and monitor care plans employing a variety of social work interventions, adjusting therapeutic approaches as necessary to fit the diverse needs of patients.

• Collaborate extensively with a multidisciplinary healthcare team to ensure patient-centered planning, incorporating input from patients and families to actively engage them in their own care.

• Facilitate discharge planning conversations, coordinating with consulting physicians, insurance representatives, and other healthcare providers to create an effective discharge roadmap for each patient.

• Provide education and counseling services that empower patients and families to adapt to the challenges posed by illness, healthcare navigation, and life transitions.

• Utilize crisis intervention strategies and facilitate both brief and long-term individual, group, and family therapies, depending on patient needs.

• Serve as a knowledgeable resource for colleagues, offering insights and guidance on psychosocial oncology practices and supporting their development through consultations or training sessions.

• Clinically assess oncology patients for signs of depression and anxiety, providing timely short-term counseling to alleviate immediate emotional distress.

• Be available for on-call rotations to address after-hour patient needs, ensuring continuity of care outside standard office hours.

• Represent the center by presenting at educational conferences, showcasing our psychosocial oncology practices to peers and the broader healthcare community.

• Engage in community outreach efforts by educating and consulting on cancer survivorship programs and resources, ultimately fostering a stronger support system for cancer patients and families.

Compensation and Benefits\n

The annual salary for this position ranges from USD 70,000 to 100,000, commensurate with experience and qualifications.
