Job Description
TITLE
Transplant Coordinator - Nursing

SCHEDULE
12-hour variable shifts, Monday-Friday, 7:30 AM - 5:00 PM; no major holidays or weekends

REQUIREMENTS
- 3+ years of transplant coordinator experience preferred
- Strong ambulatory nursing experience required
- Experience coordinating pre- and post-transplant care, reviewing labs, patient education, triage, multidisciplinary coordination, and managing medically complex patients
- Experience with kidney transplant population and Epic preferred
- BLS certification required

ROLES & RESPONSIBILITIES
- Screen patients for transplant eligibility and coordinate necessary testing and appointments
- Provide patient education regarding pre- and post-transplant care
- Manage medication protocols for transplant patients
- Collaborate with multidisciplinary teams to ensure comprehensive patient care
- Monitor and follow up on patients' clinical progress throughout their care journey
