Job Description
Job Title: Licensed Vocational Nurse (LVN)

Profession: LVN

Experience: Preferred experience in nursing or related field

License: Licensed Vocational Nurse (LVN) licensure in the state of practice is required

Certifications: Basic Life Support (BLS) Health Care Provider certification is required

Must-Have: Valid Driver's License and must be at least 21 years of age or older is preferred

Description:
Our home health and hospice agencies serve the entire community and offer personalized in-home care.


Job Summary:
Performs procedures that require specialized training for general nursing care and related services for assigned patients, under the direction of a Registered Nurse or Medical Doctor, in accordance with established policy.


Participates in the planning for and provision of patient care.


Uses knowledge of asepsis in the performance of treatments, such as sterile dressings, irrigations, and cauterizations.


Provides functional direction to ancillary personnel.


On-call duties will be assigned based on operational needs.


If assigned by leadership, the incumbent is required to perform on-call duties.


Job Requirements:
Education and Work Experience: High School Education/GED or equivalent is preferred.


Bachelor's Degree or equivalent combination of education and related experience is preferred.


Essential Functions:
Provides care for assigned patients under the supervision of a Registered Nurse according to established standards and nursing process.


Performs routine visits based on departmental specific policy and procedures.


Assists in the development of a prioritized interdisciplinary plan of care for each patient.


Implements and coordinates the plan of care utilizing appropriate organizational skills and reporting mechanisms.


Implements individual teaching plans, including Patient Teaching Protocols and discharge Plans, based on mutual goal setting and with emphasis on patient decision-making.


Performs assigned clinical procedures such as phlebotomy, injections, and drug screens.


Administers medications.


Orders and restocks supplies as assigned according to established par levels.


Maintains neatness and cleanliness in assigned areas.


Performs other job-related duties as assigned.
