Our Nurse Practitioners and Physician Assistants deliver specialized Cardiology and Pulmonology care directly to patients in Skilled Nursing, Long-Term Care, and Post-Acute Care communities, helping improve outcomes for medically complex patients while building meaningful relationships with those they serve.If you're looking for clinical autonomy, physician collaboration, and the opportunity to make a lasting impact every day, we'd love to meet you.Assigned TerritoryThis position serves patients throughout the St. Paul, MN market, providing specialty care at Skilled Nursing, Long-Term Care, and Post-Acute Care facilities located in St. Paul, St. Louis Park, Roseville, MN, and surrounding communities.
