At Nexus, we are committed to clinical excellence, innovation, and making a meaningful difference in the lives of the patients and families we serve.Position SummaryUnder the supervision of the Director of Pharmacy, the Pharmacist is responsible for overseeing all pharmacy operations within the hospital, ensuring compliance with DNV, CMS, state, and federal regulations. This role plays a critical part in medication management, clinical collaboration, and patient safety while supporting high-quality pediatric care in an inpatient setting.Job Specific ResponsibilitiesClinical & Operational ExcellenceReview, interpret, and process physician medication orders accurately and efficientlyPrepare, dispense, and monitor medication therapies in accordance with applicable laws and best practicesMonitor drug therapy, including lab values and levels; recommend or implement appropriate adjustmentsConduct medication reconciliation and ensure accurate documentation in the Medication Administration Record (MAR)Ensure all medication charges and reports are completed accurately and dailyMaintain pharmacy operations, including Pyxis system management and drug database accuracyCollaboration & CommunicationCollaborate with physicians and nursing staff to optimize medication therapy and formulary useCommunicate drug-related information clearly to patients, families, and interdisciplinary team membersPartner with providers to identify cost-effective medication alternatives when appropriateActively participate in team conferences and interdisciplinary care planningPatient & Staff EducationProvide medication education to patients and families and document all interactionsDeliver in-services and ongoing education to nursing and clinical staffSupport orientation, training, and competency validation of pharmacy and nursing personnelRegulatory Compliance & Quality ImprovementEnsure compliance with DNV, CMS, state, and federal pharmacy regulationsMaintain a clean, organized, and inspection-ready pharmacy environmentParticipate in Performance Improvement (PI) initiatives to enhance service quality and efficiencyAttend and contribute to monthly department and staff meetingsProcess & Systems ManagementFollow established protocols, including:Automatic therapeutic substitutionsIV to PO/PT conversionsManage Pyxis system functions, including:Adding medicationsEnsuring accurate medication descriptionsAdding and maintaining nursing user accessReconcile reports including discrepancies and overridesProfessional ResponsibilitiesProvide on-call pharmacy coverage as neededCommunicate effectively with vendors and external partnersAddress and resolve concerns from patients, families, and staff professionallySupport departmental goals and perform additional duties as assignedPosition QualificationsEducationDoctor of Pharmacy (PharmD) or Bachelor of Pharmacy from an accredited College of PharmacyAdvanced degree or completion of an accredited hospital pharmacy residency preferredExperienceExperience in a hospital or pediatric setting preferredStrong knowledge of DNV, CMS, and state regulatory requirementsLicensure & CertificationCurrent, active license as a Registered Pharmacist in the state of TexasIV Certification Preferred
