Summary: Vighter is recruiting Wound care, Ostomy and Continence Registered Nurses (RN) to provide exceptional patient care at Brooke Army Medical Center in San Antonio, TX. The RN provides the full range of nursing services within the scope of practice related to this specialty and ensures that all JCO standards for care are met during performance of patient care. RNs provide complex and comprehensive professional nursing services within the professional scope of practice for a variety of diagnoses for patients of all ages. Principle Accountabilities: Perform comprehensive assessments of acute and chronic wounds, including pressure injuries, diabetic wounds, surgical wounds, and skin breakdown. Develop individualized wound‑care treatment plans based on wound type, stage, drainage, tissue condition, and patient needs. Perform and manage wound treatments, including dressing changes, wound cleansing, packing, and negative‑pressure wound therapy when indicated. Assess patients for pressure‑injury risk and implement prevention strategies. Evaluate ostomies and peristomal skin for complications such as irritation, leakage, infection, or retraction. Select and apply appropriate ostomy appliances, barriers, pouches, and accessories. Educate patients and caregivers on ostomy care, pouch changes, skin care, nutrition, and lifestyle management. Provide continence assessment and management for urinary and fecal incontinence. Recommend appropriate skin‑care products and interventions to prevent moisture‑associated skin damage. Monitor healing progress and modify treatment plans as needed. Collaborate with physicians, surgeons, dietitians, physical therapists, case managers, and bedside nurses. Make recommendations regarding specialty beds, pressure‑relieving devices, support surfaces, and other wound‑care equipment. Provide consultation and education to nursing staff regarding wound, ostomy, and continence management. Document assessments, treatments, patient responses, and progress accurately in the medical record. Participate in multidisciplinary rounds and develop discharge plans for ongoing wound or ostomy care. Coordinate follow‑up care and referrals to outpatient wound or ostomy services when needed. Promote evidence‑based practices, infection prevention, and patient safety. Attends and participates in a variety of meetings and task force groups to improve quality and outcomes, communicate issues, obtain approvals, resolve problems, and maintain a specific level of knowledge of new developments, new task efforts, and new policy requirements. Functional proficiency and ability to document patient‑centered health care in the required electronic health record or electronic medical record, in the required format, in a timely manner, and in compliance with contract policy and guidance. Knowledge of regulations (HIPAA/Privacy Act) regarding the confidentiality of patient medical records and information as well as Personally Identifiable Information (PII). Performs other duties that may be assigned from time to time. Knowledge and Skills: Must have exceptional customer service skills. Employees shall be technically proficient in the skills necessary to fulfill the job requirements, including ability to speak, understand, read and write English fluently. Ability to work in a fast-paced changing environment with limited supervision Excellent organizational, scheduling and prioritization skills. Education: Associates or Bachelor of Science in Nursing from an accredited university. Qualifications: Experience: Minimum of 2 (two) years of experience as an RN in the last 4 (four) years working as a wound ostomy and continence nurse (WOC). Wound Ostomy Continence Nurse Credentialing Board (WOCNCB). Wound Ostomy Continence Nurse Education Program (WOCNEP). Shall have graduated from an accredited WOCN education program. Must have and maintain certification in Basic Life Support or Advanced Cardiac Life Support (ACLS) through the American Heart Association. Strong communication and interpersonal skills. Commitment to patient-centered care and evidence-based medicine. Ability to work independently while being a collaborative team player. Duty Hours / On-call Requirements The expected duty hours for this position are 8 hours per day and 5 days per week. However, this key position supports 24/7/365 operations which may require work to be conducted in the day, at night, on weekends, and during holidays. This position has availabilities to work day, evening and night shifts. Availability during weekends and holidays is a possibility. Advanced notice will be provided for scheduling purposes.
