Visiting Physical Therapist (PT), Day Shift, Home Health - Southern Maryland

Adventist HealthCare Home Health is hiring a Physical Therapist (PT) to serve our Southern Maryland Territory, covering Upper and Mid Prince George County. Sign‑On Bonus Eligible candidates may receive up to a $15,000 sign‑on bonus. Current AHC employees transferring within or between entities, and employees who have terminated employment with AHC and are re‑hired within six months, are not eligible. Responsibilities Perform initial examination and evaluation to establish impairment/diagnosis, prognosis, and plan of care before intervention. Provide or direct and supervise therapy intervention consistent with examination, evaluation, diagnosis, prognosis, and plan of care. Involve patient/client and appropriate others in planning, implementation, and assessment of the plan of care. Re‑examine the patient/client as necessary during a care episode to evaluate progress or change and modify or discontinue the plan of care accordingly. Plan for discharge, considering goal achievement and expected outcomes, and arrange appropriate follow‑up or referral. Communicate, coordinate, and document all aspects of patient/client management with care team members. Supervise and direct patient‑care activities of assigned rehab aides, office assistants, and volunteers. Manage schedule to accommodate census and patient load, notifying management of caseload issues. Attend team meetings and effectively communicate patient status using the medical record or other treating therapists. Maintain individual professional development activities. Qualifications Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program. Preferred 1‑year site‑specific experience. Active Maryland license. Valid driver’s license and agency insurance requirements. Active American Heart Association Basic Life Support (BLS) certification. Work Schedule Full‑time, 40 hours per week, Monday through Friday, 8:00AM–5:00PM. Weekend requirements: once per month. Holiday requirements: rotating within the team.
