Onsite Referrals & Care Coordination Specialist

Neighborhood Healthcare is seeking a Referrals Specialist to coordinate all referrals and follow-up care for PACE participants. The role handles in-house and outside referrals, scheduling, and transportation coordination, ensuring timely processing and documentation in EMR. The position is Monday–Friday, 8:30am–5pm and fully onsite, with a strong emphasis on accurate referrals, timely follow-ups, and collaboration with care teams to support patient outcomes. #J-18808-Ljbffr Neighborhood-Healthcare
