Transformative Journeys Therapy is seeking a dedicated and compassionate Psychiatric Nurse Practitioner (PMHNP) to join our dynamic team. Work Environment

The work is situated onsite in Jersey City and primarily focuses on home care; thus, the atmosphere emphasizes close collaboration among healthcare providers within our state-of-the-art facility. You will have regular working hours from 9 AM to 5 PM, allowing for a structured, balanced work-life routine.

Key Responsibilities

• Conduct thorough psychiatric assessments to accurately diagnose mental health conditions, utilizing standardized tools and techniques.
• Provide detailed medication management, ensuring the efficacy of prescribed treatments and making necessary adjustments based on patient feedback and clinical indicators.
• Collaborate seamlessly with primary care providers, therapists, and case managers to develop integrated care plans that holistically address patients’ physical and mental health needs.
• Offer timely crisis intervention services and emotional support to patients experiencing acute distress, employing trauma-informed care principles.
• Engage in community outreach initiatives to promote mental health awareness and resources, thereby reducing stigma and fostering public education.
• Deliver therapeutic support through counseling sessions and psychoeducational programs tailored to individual patient needs.
• Maintain accurate and confidential patient records in compliance with HIPAA regulations, ensuring high-quality documentation practices.

Patient Population / Client Focus

You will work with a diverse population, including children, adults, and families facing various mental health challenges such as anxiety, depression, PTSD, and other emotional disorders. Our approach acknowledges the cultural and social backgrounds of each patient, ensuring personalized care tailored to unique situations, thereby fostering an environment where every patient feels understood and valued.

Qualifications

• Completion of a Master’s degree in Nursing with a specialization as a Psychiatric Nurse Practitioner.
• Current state licensure as an Advanced Practice Registered Nurse (APRN) with a psychiatric specialization.
• Strong clinical skills in psychiatric assessment, intervention, and treatment planning.
• Ability to communicate effectively and empathetically with patients from diverse backgrounds.

Preferred Experience

• Experience working in community health centers or similar environments that focus on integrated care.
• Background in trauma-informed care practices is highly desirable.
• Familiarity with electronic health records (EHR) systems.

Compensation and Benefits

The salary for this position ranges from USD 200,000 to 250,000 per year, based on experience and qualifications.
