Our facilities function as Skilled Nursing Facilities (SNFs) that offer:
- Long-term care for residents needing ongoing skilled support and compassionate daily assistance
- Short-term, sub-acute rehabilitation for patients recovering from surgery, illness, or injury
- Collaborative care through a team approach involving nursing, therapy, and medical staff
- A diverse patient demographic, including those with orthopedic, neurological, cardiopulmonary, and post-acute needs
As a therapy professional, you will play a crucial role in helping patients achieve functional improvements and enhance their quality of life in a supportive, patient-focused setting.
Position SummaryThe Speech Language Pathologist (SLP) delivers direct rehabilitation services in speech therapy to patients referred by physicians, adhering to all relevant laws, regulations, and organizational standards.
Education, Experience, and Licensing Requirements- Must be a graduate of an accredited speech therapy program (minimum MS or MA in SLP)
- ASHA certification (CCCs) is required OR eligibility for a 9-month Clinical Fellowship Year (CFY) to obtain ASHA certification (CCCs) and/or meet ASHA credentialing criteria. Maintenance of ASHA credentials is mandatory.
- Current registration/licensure in the applicable state is required, with an active license in good standing throughout employment.
- Preferred: One year of experience in a post-acute care or similar setting.
- CPR certification is required upon hire or must be obtained during orientation, and it must remain current throughout employment.
- Engage in community health initiatives/projects as appropriate
- Proficient in Microsoft Word, Excel, and email communication
- Collaborate with patients, families, and support departments to effectively plan for patient needs
- Exhibit proper body mechanics at all times
- Demonstrate the ability to make independent decisions when necessary
- Knowledgeable about practices, procedures, and regulations governing operations in post-acute care facilities
- Implement and interpret departmental programs, goals, objectives, policies, and procedures
- Perform competently in all areas, including patient rights, safety, and sanitation
- Maintain professional relationships with colleagues, vendors, and others
- Ensure confidentiality of all proprietary and sensitive information
- Understand and adhere to company policies, including those related to harassment and compliance
- Exhibit integrity and professionalism by following the organization's Code of Conduct and completing required compliance training
- Establish, assess, and adjust realistic, measurable, timely, and functional goals
- (SLP CCC only) Oversee and evaluate care provided by SLP CFYs
- (SLP CFY only) Follow speech therapy treatment plans under the supervision of the SLP CCC, in accordance with federal and state guidelines
- Document patient care accurately and in a timely manner
- Utilize therapy software effectively and accurately
- Provide excellent customer service and maintain a positive attitude towards patients
- Assist in the safe evacuation of patients when necessary
- Demonstrate reliable attendance
- Employ reasoning skills and sound judgment in all situations
- Communicate and collaborate effectively within an interdisciplinary team
- Engage in physical activities such as sitting, standing, bending, lifting, pushing, pulling, stooping, walking, reaching, and moving intermittently during work hours
- Read, write, speak, and comprehend the English language
