Full-Time position.Location: Hampden & Chambers Clinic - Aurora, CO.General Purpose: In order to ensure efficient, quality health care services, the Nurse Practitioner or Physician Assistant will provide medical screenings and primary care to our patients.Essential Duties/Position Responsibilities:Provides direct health care services to patients.Offers clinical consultation and arranges referrals to other providers and clinical staff.Reviews lab and x-ray results of other providers patient care results.Prescribes medications to patients, including counseling patients regarding the proper use and contraindications of medications as well as detect and diagnose adverse drug reactions.Evaluates medical charts to determine appropriate patient care follow up and education regarding acute and chronic illnesses.Accurately and completely charts information regarding patient interactions into the Electronic Health Record (EHR).Participate in the STRIDE after-hours telephone call system.Attends all staff meetings and all other meetings as assigned.STRIDE Values:Community: All are welcome at STRIDE - our patients are the center of our work.Trust: Our patients feel safe, empowered, and supported.Value: We respond to patients' needs by providing effective, affordable care.Compassion: We treat patients with kindness, empathy, and dignity.Education and Experience:Required: Master's degree in nursing from an accredited Nurse Practitioner Program and/or Master's degree in Physician Assistant studies.Required: Licensed or eligible for licensure in Colorado (APN or PA).Required: Board certification.Required: Active BLS Certification.Previous experience as a nurse practitioner/physician assistant in a clinical setting is preferred.Two years post licensing preferred.
