About This Role The RN/Case Manager will oversee case management for a designated group of patients, ensuring appropriate levels of care and collaborating with an interdisciplinary team to enhance patient outcomes. Highlights Pay: $63.28/hour Location: New York, NY Schedule: Part-time, Day shift What You'll Do Review new admissions to identify case management needs. Perform continued stay reviews to justify inpatient stays. Collaborate with physicians to expedite diagnostics and treatments. Document clinical reviews in the Case Management Documentation system. Assess patients' clinical, psychosocial, and functional status. Identify variances affecting length of stay and discharge planning. Conduct follow-ups on treatment delays and reporting results. Implement and coordinate activities to facilitate patient care. Requirements Education Bachelor's Degree in Nursing; Master's preferred Experience 3-5 years experience as an RN in an acute care setting Previous experience in homecare, long-term care, or utilization review preferred Licenses & Certifications Current NYS RN nursing license CCM certification preferred PRI Certification (to be obtained within 1 year) Basic Life Saver (BLS) certification required About Mount Sinai The Mount Sinai Health System is one of the largest academic medical systems in the New York metro area, dedicated to delivering outstanding patient care and fostering an inclusive environment for all employees. #J-18808-Ljbffr IntelliResume
