This role allows you to work remotely, providing you with the necessary work-life balance while still being actively engaged with our hospice clients. Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to evaluate clients' mental health status, strengths, challenges, and support systems.

• Provide individualized therapeutic interventions, including individual, family, and group therapy sessions, utilizing evidence-based modalities such as cognitive-behavioral therapy, solution-focused therapy, and trauma-informed care.

• Develop, implement, and regularly update individualized treatment plans that cater to clients' specific needs and goals.
\n
• Collaborate with psychiatrists, psychologists, and other healthcare professionals to ensure an integrated and client-centered approach to care.

• Facilitate group therapy sessions and psychoeducational workshops to foster social support and skill development among clients.
\n
• Provide crisis intervention services and safety planning as required, ensuring clients receive timely and effective support during critical times.

• Maintain meticulous and timely documentation in accordance with ethical and legal standards, utilizing our electronic health record (EHR) system.

• Participate in regular team meetings to discuss client progress, share insights, and update treatment plans as necessary.

Patient Population / Client Focus\n

The client population served in this role includes individuals experiencing a range of mental health conditions, such as depression, anxiety disorders, and trauma-related disorders, particularly within our hospice program. In this sensitive environment, you will work closely with clients and their families, addressing both emotional and psychological aspects of coping with end-of-life issues, providing them with the support and tools needed to navigate these challenging experiences.

Qualifications\n

• Master's degree in Social Work (MSW) from an accredited institution.

• Active state licensure as a Licensed Clinical Social Worker (LCSW).

• Strong knowledge of evidence-based therapeutic modalities and demonstrated effectiveness in applying such approaches in clinical practice.

• Excellent interpersonal skills, with the ability to build rapport quickly and effectively with clients.

Compensation and Benefits\n
