IMPaCT Care in Pittsburgh seeks a Director of CHW Services to lead a team of one Manager and 10 CHWs serving hundreds of clients annually. This on-site role partners with Highmark Health to strengthen community health initiatives and deliver high-quality patient support. You will recruit, train, and supervise staff, develop external partnerships, monitor outcomes with data, and drive program excellence.
