Direct Care - Clinical Therapist/Social Work/Case Worker As a Service Coordinator, you are more than a case manager- you are the person helping individuals turn their goals into real opportunities and meaningful progress. You will work directly with individuals enrolled in HCS, TxHml, and GR CFC programs, helping them identify their needs, advocate for their preferences and build a life that reflects their personal goals and independence. Every conversation, connection and service you coordinate has the power to create lasting impact. In this role, you will be the trusted guide ensuring services truly align with each individual’s Person- Directed Plan (PDP). From connecting people to valuable community resources to monitoring and coordinating essential support, you will play a vital role in delivering consumer-driven services that empower individuals and families throughout Tarrant County.
