Position Overview

\n

We are seeking a highly experienced and dedicated Nurse Practitioner (NP) to join our clinical team at a rehabilitation center in Staten Island, NY. The ideal candidate will have extensive clinical experience, strong patient assessment skills, and the ability to manage complex medical needs in a rehabilitation or long-term care setting.

\n

The successful candidate will collaborate with physicians, nursing staff, and other healthcare professionals to provide high-quality, patient-centered care and support patients throughout their recovery and rehabilitation journey.

\n

Key Responsibilities

\n

• Conduct comprehensive patient assessments, medical evaluations, and physical examinations.
\n
• Develop, implement, and monitor individualized treatment and care plans.
\n
• Manage chronic conditions, acute medical concerns, and complex patient needs.
\n
• Monitor patients’ clinical progress and adjust treatment plans as necessary.
\n
• Prescribe medications and order diagnostic tests in accordance with New York State regulations and facility protocols.
\n
• Coordinate care with physicians, registered nurses, therapists, and multidisciplinary teams.
\n
• Maintain accurate and timely clinical documentation, progress notes, and medical records.
\n
• Identify potential complications and initiate appropriate clinical interventions.
\n
• Educate patients and their families regarding treatment plans, medication management, and ongoing care.
\n
• Ensure compliance with applicable healthcare regulations, clinical standards, and facility policies.
\n

Qualifications

\n

\n
• Active, unrestricted Nurse Practitioner license and certification to practice in New York State.
\n
• Current national NP certification in the appropriate specialty.
\n
• Extensive clinical experience, preferably 5+ years as a Nurse Practitioner.
\n
• Previous experience in rehabilitation, long-term care, skilled nursing, subacute care, or a similar clinical setting is strongly preferred.
\n
• Strong knowledge of chronic disease management, patient assessment, and care coordination.
\n
• Excellent clinical judgment, communication, and decision-making skills.
\n
• Ability to work independently while collaborating effectively with a multidisciplinary team.
\n
• Current BLS certification; additional certifications as required by the facility.
\n

Preferred Candidate Profile

\n

We are particularly interested in senior-level Nurse Practitioners with a proven track record of managing medically complex patients and providing ongoing care in rehabilitation or long-term care environments.

\n

Candidates with experience in skilled nursing facilities, post-acute rehabilitation, or geriatric care are encouraged to apply.
