Case Manager Supervisor

Job Summary The Case Manager Supervisor is responsible for the day-to-day management of the Case Management staff. This team is responsible for providing comprehensive care for homeless children and their families. This position will utilize assessment tools to focus on basic needs, help stabilize the immediate crisis, aid in establishing a long-term plan for recovery, and re-assess and measure success. The goal of this position is to improve basic needs and social connectedness and to be the service advocate for homeless families. Job Responsibilities Provide daily supervision to the Case Management staff. Provide case conferencing assistance and support to program Case Managers and volunteers. Assign cases; assist in referrals and case management decisions. Provide crisis intervention, case conferencing and support to program supervisors, staff and volunteers. Audit case files, service plan notes and other documentation of the STC/LIN Case Management staff to ensure all paperwork is present and complete. Communicate changes in procedures and policy to all Case Management staff. Oversee the adherence to program standards and ensure case files are accurate and complete by conducting regular audits of case records. Implement and train staff on appropriate administration policies, services, structures and controls for the effective and efficient delivery of services. Assist in forming collaborative relationships with community groups; partner with agencies and groups to eliminate gaps in services and reduce duplication of offered services; secure and maintain community support for Social Services programs. Direct and evaluate the performance of the Case Management staff. Conduct presentations and trainings in the community as needed. Work closely with Directors of Programs to develop new policies and procedures consistent with the agency mission statement, agency policies and procedures that align with best practice standards. Attend staff meetings, training sessions as requested and any other meetings requested by the Director of Program Services and the Executive Director. Attend necessary training and continuing education programs to ensure the highest level of efficiency. Maintain confidentiality for all personal recipient information and assure that program participant’s rights are protected. Other duties as assigned. Job Requirements Understanding of needs of persons experiencing homelessness and those who are living in poverty. Case management methods, principles and techniques. Community and agency resources. Experience with and ability to use: computer (Word, Excel, Word, Excel and Bell Data), fax and telephone. Exercise good judgment and use good organizational skills in evaluation situations, making referral decisions and following through. Work harmoniously with applicants, recipients, general public and other employees. Work in and promote a team environment Ability to: Plan and administer a large-scale service program. Establish clear departmental goals and objectives and follow through on their attainment. Exercise good judgment and use good organizational skills in evaluation situations, making decisions and following through. Effectively communicate verbally and in writing. Work harmoniously with applicants, recipients, general public and other employees.Plan, organize, assign, delegate and prioritize work. Assist in grant formulation and be familiar with the compliance process. Use strong listening and conflict resolution skills To manage high stress and emergency situations professionally and appropriately. To demonstrate ability to be flexible and tolerant with persons of diverse backgrounds, economic status, and life experiences. Use strong organizational skills with the ability to manage multiple tasks. Ability to pass a background check.
