Houston Hospice is seeking an RN Field Case Manager for the North of I-10 area in Houston, TX. This full-time professional role oversees overall case management and coordinates care with clients, families, and providers to ensure timely access to primary physicians or psychiatrists. You will develop and maintain case plans, document per state regulations, and train staff on referral processes while collaborating with RNs, LPNs, and other stakeholders to support compassionate end-of-life care. #J-18808-Ljbffr Houston Hospice
