Key Responsibilities\n

• Conduct comprehensive assessments of seniors’ psychosocial needs, utilizing a variety of evaluation techniques to understand their emotional, mental, and social conditions.

• Develop personalized care plans in collaboration with clients and families, ensuring each plan aligns with specific goals, preferences, and available resources.

• Provide ongoing counseling and emotional support to seniors and their families, helping them navigate life transitions, such as aging, illness, and loss.

• Advocate for clients’ rights and facilitate access to community resources, ensuring that they are informed and receive the necessary support services.

• Engage with interdisciplinary teams to enhance overall care, regularly communicating updates and plans of action with nurses, caregivers, and other health professionals.

• Conduct regular home visits to monitor clients’ well-being and living conditions, identifying any concerns or issues that may affect their safety and quality of life.

• Facilitate family meetings, assisting in conflict resolution and enhancing communication among family members involved in the client's care.

• Educate clients and families about community resources, support groups, and programs available to enrich social engagement and quality of life.

• Maintain accurate and detailed documentation of assessments, care plans, and progress notes in compliance with organizational and ethical standards.

• Stay current with best practices and regulations in social work, participating in professional development and networking opportunities.

Qualifications\n

• A Bachelor’s degree in Social Work (BSW) from an accredited institution, with a current and valid state licensure as a Social Worker (LCSW) required.

Compensation and Benefits\n

The position offers a competitive salary range of USD 60,000 – 100,000 per year, commensurate with experience.
