Key Responsibilities\n

As a Licensed Clinical Social Worker, your role will include a variety of responsibilities that are essential to our clients' well-being:

• Conduct thorough evaluations and assessments to determine the psychosocial needs of seniors and their families.

• Participate in the development and implementation of Individualized Family Service Plans (IFSP) tailored to each client’s unique situation.

• Regularly monitor client outcomes and progress as an active member of the interdisciplinary team.
\n
• Provide emotional support and resources to families to help them cope with grief, loss, and the emotional aspects of hospice care.

• Ensure compliance with all relevant federal and state regulations regarding social work practices.

• Build and maintain positive relationships with clients, families, and other team members to create a healing environment.

• Utilize evidence-based practices to administer interventions and recommend appropriate services to enhance overall well-being.

Patient Population / Client Focus\n

Your primary focus will be on seniors receiving hospice care, a demographic that requires unique sensitivity and understanding. You will work with clients who are often experiencing complex medical and emotional situations, necessitating a compassionate and informed approach to your interventions. The role will involve engaging with clients and their families to provide support, address concerns, and facilitate connections with relevant resources.

Qualifications\n

• A Master’s degree in Social Work (MSW) or a related field is required.

• Licensed Clinical Social Worker (LCSW) certification in the state of Florida is mandatory.

• Strong communication skills, both verbal and written, which are essential for effective interaction with clients and team members.

• Demonstrated organizational and planning skills to manage multiple clients and priorities effectively.

• A comprehensive understanding of the assessment, diagnosis, and treatment of geriatrics and hospice populations.

Preferred Experience\n

• At least 2 years of clinical experience as a Licensed Clinical Social Worker is preferred.

• Experience in working with hospice clients and their families, particularly in a remote or outpatient setting.

Compensation and Benefits\n
