Key Responsibilities\n

• Conduct comprehensive psychosocial assessments of patients, using your training and expertise to diagnose psychological issues and formulate suitable treatment plans.

• Employ evidence-based therapeutic techniques, including solution-focused therapy, motivational interviewing, and problem-solving treatment to facilitate behavioral change and support patient care goals.

• Maintain meticulous documentation of patient progress in accordance with regulatory standards, utilizing electronic health record systems for tracking treatment plans and outcomes.

• Ensure the administration of psychiatric medications aligns with best practices by monitoring adherence, side effects, and overall efficacy, coordinating with medical practitioners as necessary.

Patient Population / Client Focus\n

This role primarily involves supporting hospice patients facing end-of-life issues, along with their families, addressing emotional distress, grief, and adjustment to changing circumstances. Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.

• Possess a current, valid, and unencumbered Licensed Clinical Social Worker (LCSW) license allowing for independent practice in Florida.
