This LCSW position operates in a remote setting, allowing flexibility while connecting with clients primarily through virtual platforms. Key Responsibilities\n

• Conduct thorough evaluations and assessments to determine the emotional and social needs of clients and their families.

• Collaborate in the development and implementation of Individualized Family Service Plans (IFSP), ensuring that the family’s preferences and priorities are central to the approach.

• Monitor and track patient outcomes and the impact of interventions while making necessary adjustments to care plans based on ongoing assessments.
\n
• Provide information, resources, and emotional support to families to enhance their understanding of hospice care services and facilitate a smooth transition during difficult periods.
\n
• Employ effective communication strategies to establish rapport and trust with children and families, fostering a nurturing environment conducive to healing.
\n
• Utilize evidence-based practices and demonstrate commitment to Natural Learning Environment Practices in all interactions.
\n
• Ensure compliance with all Federal and State regulations governing social work practices and hospice services.

Patient Population / Client Focus\n

This role primarily serves children and their families involved in hospice care. You will address the unique emotional and developmental challenges faced by these families, many of whom are experiencing significant transitions and grief. By providing tailored support, you will help families navigate their feelings, improve their coping mechanisms, and access necessary resources effectively during this critical time in their lives.

Qualifications\n

• A Master's Degree in Social Work (MSW) or a Doctorate in Social Work (DSW) is required.

• Current licensure as a Licensed Clinical Social Worker (LCSW) issued by the Department of Public Health in Florida.

• A valid driver's license is necessary for any required on-site family interventions.

• A minimum of one year of documented experience in providing social work services, particularly with children and families in early intervention or hospice settings.

• Proficient in adapting to various learning styles and demonstrating flexibility in approach to meet individual family needs.

• Excellent organizational and time management capabilities, critical for managing case loads effectively.

Compensation and Benefits\n
