Crisis Intervention & Care Navigator
MultiCare Health System
The Crisis Care Coordinator at MultiCare Health System provides follow up services for individuals seen in crisis and helps coordinate referrals and warm hand offs to appropriate services in the community to stabilize and/or mitigate future crisis. You will assess needs, evaluate risk, and collaborate with law enforcement and first responders to educate and support diversion from unnecessary emergency department visits. #J-18808-Ljbffr MultiCare Health System
