Key Responsibilities\n

In your role as a Licensed Clinical Social Worker, you will be responsible for a variety of tasks that are crucial to patient care:

• Facilitate psychoeducational and therapy groups that provide patients with essential coping skills and emotional support.

• Initiate discharge planning on the day of patient admission, ensuring a smooth transition to outpatient care.

• Provide patients with discharge sessions to review their progress, coping strategies, and confirm understanding of their transition plans.

• Maintain open lines of communication with the Utilization Review Department and Business Department regarding authorization and payment processes.
\n
• Confirm discharge appointments with patients and their families to ensure a seamless continuation of care.

• Collaborate with referral sources and outpatient providers to ensure ongoing support and continuity in treatment.

Qualifications\n
