Nurse Navigator RN Outpatient Oncology Advocate Good Shepherd Hospital Per Diem Join our Outpatient Oncology department at Advocate Good Shepherd Hospital, where you will find a supportive, collaborative and highly engaged team dedicated to safely serving our special patients with compassion. Responsibilities Coordination and implementation of strategies to ensure quality, efficacy, and cost effectiveness for the oncology patients in multiple settings. Deliver cancer care across the continuum, from diagnosis through treatment, and implement transition to survivorship and end‑of‑life care. Using the nursing process, provide patient and family insight with appropriate education and resources to facilitate informed decision‑making and timely access to quality health and psychosocial care throughout all phases of the cancer continuum. Critically think and clinically assess patients providing support and education. Manage the complexity of the cancer diagnosis. Communicate and collaborate with all multidisciplinary team members. Orientate patient to the hospital and outpatient settings as well as the organization. Assess patient needs. Review distress screening for referrals. Communicate and collaborate with the treating physician. Communicate and collaborate with the multidisciplinary team. Receive and respond to new patient referrals. Develop and report outcome metrics to drive program development. Assess and implement activities that support the standards related to Children’s Oncology Group (COG). Review and strategize activities that support the community needs assessment of site‑specific results. Provide emotional support for patients; advocate on behalf of the patient; empower the patient to self‑advocate. Maintain own knowledge, ability, skills, and competencies; continue to role model organizational behaviors of excellence. Licensure Registered Nurse (RN). Education/Experience Required BSN required. Typically requires 5+ years experience in clinical specialty (Oncology). Knowledge, Skills & Abilities Required Global perspective of changing health care environment. Ability to prioritize work. Basic computer skills. Effective interpersonal communication skills. Current Illinois RN license. CPR certification. OCN Certification or Nurse Navigator Certification obtained within 2 years of hire. Physical Requirements and Working Conditions Flexible hours according to customer needs. Ability to demonstrate a commitment to patient‑focused care. Ability to prioritize independently and respond to multiple simultaneous requests. Ability to work in a fast‑paced environment with established time constraints and in difficult situations. May be exposed to hazardous materials and life‑threatening diseases. Ability to perform lifting/transfer activities related to patients (up to 50 lbs. may be required); may lift up to 35 pounds without assistance; for lifts over 35 lbs. use patient handling equipment with at least one other associate when available. Must be able to travel to visit physicians and other practices within our region.
