SUMMARY

The Assertive Community Treatment (ACT) Program Care Manager position is responsible for all the functions of ACT Program services, including advocacy, assessment, linking, coordination, and monitoring of services for an assigned caseload of individuals with a primary diagnosis of mental illness. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following: * The ACT Care Manager is responsible for developing a thorough understanding of Michigan mental health code and standards, as well as the standards of funding and certifying agencies such as Department of Mental Health, Medicaid and CARF, regarding ACT Services. * The ACT Care Manager is part of a team that provides the on-going screening and assessment of individuals referred whose presenting problems, diagnosis and/or treatment needs suggest a primary concern of mental illness. * The ACT Care Manager secures information such as psychological and social history contributing to the client's situation and evaluates these and the client's needs and capacities to determine least restrictive environments or services. * The ACT Care Manager initiates and coordinates professional assessments according to presenting problem and relevant standards, and is responsible for sharing and presenting assessment/treatment data to the team and as authorized and required for implementation and coordination with other agencies, courts and caretakers. * The ACT Care Manager assures as needed, but at least annually, an evaluation of the need for continued services, the development of an integrated plan of service and engages in continued compliance with documentation, billing and other requirements to obtain maximum reimbursement. * The ACT Care Manager completes the individual plan of service and quarterly reviews of progress, and integrates the preferences and satisfaction of the individual served in the treatment/training plan and goals. * Completes Locus assessments annually, quarterly and post hospital stay or as needed due to significant changes for the individual. * The ACT Care Manager makes contacts with assigned individuals and is responsible for assuring the implementation, coordination and monitoring of the plan of service in residential, employment, community and other settings. * The ACT Care Manager provides consultation and in-service training to caretakers, families and other agencies as needed to meet the specialized needs of individuals served. * The ACT Care Manager refers individuals to community resources as needed and appropriate, and encourages and supports them in their efforts to utilize integrated services and resources appropriately. * The ACT Care Manager may participate in and assist with placement planning, completing necessary forms and paperwork for placement. * The ACT Care Manager helps individuals to understand their feelings, attitudes and patterns of behavior. * The ACT Care Manager maintains ongoing communication with supervisor regarding assigned duties. * The ACT Care Manager may be called upon to attend conferences and engage in training and in-service presentations, and in public relations speaking engagements.
  • The ACT Care Manager performs crisis intervention duties, as assigned.
  • The ACT Care Manager is required to participate in after hours on call.
  • Perform other duties as assigned.

QUALIFICATIONS REQUIREMENTS

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. A.) in human services field from an accredited four-year college or university, preferably with experience working with individuals having a mental illness. Professionally licensed, certified, or registered by State or national organization to provide health care services (i.e. LMSW, LBSW LLP, LPC). Candidates/Employee must also qualify as QMHP or QMRP. Substance license preferred; staff are encouraged to apply for a MCBAP development plan.

LANGUAGE SKILLS

Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.

MATHEMATICAL SKILLS

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.

REASONING ABILITY

Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

COMPUTER SKILLS

To perform this job successfully, an individual should have knowledge of Word Processing software. Recommended typing speed of 45 wpm.
