Essential Functions:

Responsibilities- Performs outreach to patients
- Schedules appointments
- Coordinates with clinics to ensure appropriate appointments
- Tracks details on excel spread sheet
- Drill down of care gaps

Physical Demands:
While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand; walk; use hands to finger, handle or feel; and reach with hands and arms. The employee may occasionally lift up to 10 pounds.
Qualifications and Education Requirements
- High School Diploma and 1-2 years of experience
- Experience with Microsoft Office, Epic System, running reports and data mining
- Experience in Clinical Operations
- Must have strong organizational skills to effectively prioritize, coordinate, perform and complete multiple duties and assignments concurrently and in a timely manner.
- Knowledge of and proficiency in English composition, spelling, grammar, punctuation and basic math principles
- Strong presentation, interpersonal and communication skills
