Your Role & Responsibilities As a Home Health OT, you will provide skilled occupational therapy services to a diverse caseload of clients with various conditions, including: Neurological, musculoskeletal, orthopedic, and cardiopulmonary impairments Fine motor, sensory-motor, and developmental delays Traumatic injuries and swallowing disorders Key responsibilities include: Conducting comprehensive initial evaluations and developing individualized, evidence-based Plans of Care. Providing direct therapeutic interventions to improve clients' safety, independence, and functional abilities. Educating clients, family members, and caregivers on therapeutic techniques, home exercise programs, and adaptive equipment. Completing timely and accurate documentation electronically. Collaborating with physicians, nurses, and other members of the interdisciplinary healthcare team. Managing discharge planning to ensure a safe transition for the client. Qualifications of the Ideal Candidate We are looking for a dependable, energetic, and empathetic professional with excellent clinical and interpersonal skills. Required: Valid, current, and unrestricted Occupational Therapist license in the State of Wisconsin. A professional, positive, and patient-centered attitude. Strong organizational, assessment, and problem-solving skills. Excellent communication skills for interacting with patients, families, and physicians. Proficiency with technology and ability to chart electronically. A valid driver’s license, reliable vehicle, and current automobile insurance. Ability to pass a pre-employment physical, drug screen, and background check. Current TB skin test or willingness to complete one upon hire. Preferred: 1-3 years of experience in a home health setting is a plus, but not required. We welcome motivated therapists with relevant clinical experience to apply.
