Housing Case Manager

Community Missions of Niagara Frontier is seeking a Case Manager within the Mental Health Housing Services division to oversee and manage the cases of all tenants in Supportive Housing. You will help develop Tenant Individual Support Plans and ensure ongoing linkage to community service providers. This full-time role requires a Bachelor’s degree in Social Work or related field and a NYS driver’s license. You will observe, document and report tenant status to support coordinated care and housing #J-18808-Ljbffr
