Work Environment\n

This remote position allows for flexibility and work-life balance while ensuring that our clients receive continuous care. Standard work hours are Monday through Friday, 8:00 AM to 4:30 PM, though occasional evening and weekend hours may be necessary based on client needs. As a vital member of our team, your input will be valued, and you may be called upon to provide on-call support during urgent situations. Key Responsibilities\n

• Conduct comprehensive assessments of the psychosocial needs of seniors, integrating emotional, mental, and social dimensions into your evaluations.

• Develop and implement personalized care plans in collaboration with clients and their families, ensuring that their specific goals and preferences are prioritized.

• Provide individual and family counseling, offering emotional support and strategies to navigate life transitions such as aging, illness, or loss.

Qualifications\n

• Master's Degree in Social Work from an accredited institution is required.

• Current and valid LCSW licensure in the state of New York.

Compensation and Benefits\n
