Work Environment\nThis position is primarily remote, offering flexibility while maintaining a strong connection to the team and patients. Our clinic operates Monday through Friday, from 9:00 AM to 5:00 PM. The remote setting facilitates a comfortable atmosphere for both clinicians and clients, allowing for effective communication and support through technology. You will be equipped with the necessary tools, including a secure electronic health record (EHR) system, to manage patient interactions efficiently and confidentially.
Key Responsibilities\nAs a Licensed Clinical Social Worker, your key responsibilities will include:

- Conducting thorough psychosocial assessments to ascertain patients' needs, strengths, and challenges, facilitating tailored mental health care. \n
- Providing effective counseling sessions focused on disease management, behavior modification, and health education, ensuring each patient receives personalized support and guidance. \n
- Collaborating with patients and an interdisciplinary team to develop and implement individualized care plans, targeting mental health improvement and lifestyle changes aligned with patient goals. \n
- Supporting patients with chronic conditions in managing their requirements and associated mental health needs through synchronous and asynchronous goal-setting. \n
- Engaging in regular outreach and follow-ups based on patient preferences and needs, enhancing patient engagement and promoting positive behavior changes. \n
- Documenting all patient interactions and progress accurately and promptly in the EHR system, ensuring compliance with best practices and maintaining comprehensive patient records. \n

Qualifications\nTo be considered for this vital position, candidates must meet the following qualifications:
