Home Health Registered Nurse (RN) Location: San Antonio, TX and surrounding areas Schedule: Monday-Friday, 8:00 AM-4:30 PM Weekly Hours Guaranteed: 36 Hours Coverage Area: Up to a 60-mile radius surrounding San Antonio, TX On-Call/Weekend Requirement: Yes, rotating on-call and weekend schedule Mileage Reimbursement: Yes, mileage reimbursed between patient visits Position Overview We are seeking a compassionate and skilled Home Health Registered Nurse (RN) to join our growing team in the San Antonio, TX area. In this role, you will provide high-quality skilled nursing care to patients in their homes, helping them achieve optimal health outcomes while maintaining independence and comfort in their home environment. The ideal candidate is a licensed RN with strong clinical skills, excellent communication abilities, and a passion for delivering patient-centered care. Key Responsibilities Provide skilled nursing care to patients in a home health setting. Conduct comprehensive patient assessments and develop individualized care plans. Monitor patient conditions and document clinical findings accurately and timely. Administer medications, treatments, and other physician-ordered interventions. Educate patients and caregivers on disease management, medication compliance, and health maintenance. Collaborate with physicians, therapists, and interdisciplinary team members to coordinate patient care. Ensure compliance with home health regulations, policies, and documentation standards. Required Qualifications Active and unrestricted Registered Nurse (RN) license. Graduate of an accredited nursing program. Strong clinical assessment and critical-thinking skills. Valid driver's license and reliable transportation. Ability to travel throughout the assigned coverage area. Previous Home Health experience. Experience with Homecare Homebase (HCHB) software.
