Visiting Nurse Health System is looking for an experienced and mission-driven Senior Manager of Case Management - EDWP to partner with the Director of Case Management (DCM) to oversee care coordination across community-based programs. In this highly visible leadership role, you will strengthen daily operations, support team performance, and help ensure members receive coordinated, high-quality services. This in-office position is based at our Hartsfield or Marietta office, Monday through Friday.

Job Overview:

The Senior Manager of Case Management (SMCM) for Visiting Nurse Health System’s (VNHS) Elderly and Disabled Waiver Program (EDWP) manages Team Leads and case management teams, ensures high-quality member support, and aligns daily operations with organizational goals, regulatory standards, and best practices in care management. The SMCM supports interdisciplinary teamwork, monitors performance metrics, and collaborates on strategies to improve member outcomes, access to services, and continuity of care. The SMCM works with the DCM to develop and execute departmental initiatives, streamline workflows, and address barriers to member care in community settings.

The best fit communicates quickly and in detail, is comfortable picking up the phone or sending a text rather than waiting on a formal memo, and is a natural translator — turning policy and performance data into plain guidance the frontline team can act on immediately. They lead through instruction and encouragement, taking new Case Managers under their wing during onboarding and extending real autonomy only as trust is earned, one relationship at a time.

They are comfortable making the dozens of day-to-day calls this role demands — staffing, scheduling, workflow — and know instinctively when a case is too complex or too regulated to handle alone, bringing it to the Director of Case Management without hesitation. What keeps this person engaged for the long run is a busy, socially connected, ever-changing environment, along with real recognition — a title, a shout-out in a team meeting, visible credit — for the outcomes their team delivers.

What you'll be doing:

The Senior Manager of Case Management - EDWP oversees the daily operations of EDWP, providing leadership and guidance to Team Leads and Case Managers while ensuring high-quality, member-centered services. This role is responsible for operational performance, clinical oversight, staff development, regulatory compliance, and collaboration with community partners to improve member outcomes.

• Oversee daily EDWP operations, resource allocation, staffing, and program performance. 
• Lead and support Team Leads and Case Managers through coaching, consultation, performance management, and professional development. 
• Monitor key performance metrics, identify variances, and implement quality improvement and corrective action strategies. 
• Provide clinical oversight through complex case consultation, joint home visits, assessment conferences, and documentation reviews. 
• Ensure care is coordinated effectively with physicians, nursing leadership, DCH, and community stakeholders. 
• Support recruitment, onboarding, orientation, performance evaluations, and ongoing development of case management staff. 
• Promote culturally responsive, member-centered care and effective integration of medical, behavioral, and social services. 
• Support complaint resolution, clinical investigations, risk management, and documentation requirements. 
• Serve as a secondary liaison with the Department of Community Health (DCH) for the EDWP. 
• Ensure compliance with EDWP, Medicaid, Medicare, federal/state regulations, accreditation standards, and VNHS policies . 
• Promote member safety, infection control, and evidence-based practices across the program.

Minimum Qualifications:

• Bachelor’s degree in health sciences, social work, counseling, or a related field.
• Master’s degree is a plus.
• Licensed Clinical Social Worker (LCSW) is a plus.
• Licensed Master Social Worker (LMSW) is a plus.
• Certified Social Work Case Manager (C-SWCM) is a plus.
• Valid CPR certification.
• Current, valid driver’s license.
• Minimum of three years of case management experience.
• Minimum of three years of leadership experience.
• Minimum of three years of experience working with Medicaid Waiver programs.
• Minimum of three years of experience in community-based healthcare, public health, care coordination, or a similar healthcare or social services role.
• Knowledge of Medicaid Waiver policies and regulations.
• Demonstrated knowledge of KPIs and performance management.
• Strong knowledge of healthcare regulations, quality standards, and care coordination models.
• Strong understanding of community health issues and social determinants of health and access to care.
• Demonstrated proficiency using digital tools and platforms for communication, documentation, reporting, and data tracking.

• Experience working with underserved or vulnerable populations.

• Proficiency with Microsoft Office applications, internet navigation, email, computer file management, online HR services, and online training platforms. Knowledge of the Georgia Horizon system is a plus.

Salary Range: $80,000.00 - $100,000.00
