Seeking an experienced Social Services Director or Designee whose passion is supporting the holistic, emotional, and psychosocial well-being of each resident at a mid-sized Skilled Nursing Facility located in the Beverly Hills area.
Key duties include :
- Conducting psychosocial assessments
- Navigating the complex Medicare/Medicaid resources alongside families
- Participating the team care planning process
- Coordinating safe discharge plans
- Resident and family counseling & support
- Regulatory compliance and documentation
- Advocacy and grievance interventions
