Responsibilities

Ascension is looking for an APRN or PA for our Oakleaf - Primary Care Clinic in Jacksonville, FL. Responsibilities include obtaining patient history and performing assessments, ordering and interpreting diagnostic studies, performing preventative health assessments, screening, immunizations, and patient care. Direct treatment and management of health conditions, counseling and educating patients and their families/caregivers concerning preventative health, treatment options, and community resources, and documenting delivery of health care and nursing processes in accordance with specific specialty area standards and network nursing policy.

What You Will Need

Licensure / Certification / Registration: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. Nurse Practitioner credentialed from the Florida Board of Nursing obtained prior to hire date or job transfer date required. Valid state license required specific to the state in which the associate works. Education: Master's degree of Nursing required.
