You will evaluate patients at home, develop individualized care plans, and collaborate with PTAs to execute them. You will coordinate with physicians, monitor progress, and document thoroughly while maintaining autonomy with flexible hours. A CA PT license and BLS/CPR are required, with local driving and mileage reimbursement. #J-18808-Ljbffr
