Associate Staff, Medical Care at Home- House Calls

Overview Cleveland Clinic Community Care is seeking a physician to join our expanding physician home based primary care practice. Become part of a cutting edge home care team that is combining new technologies and innovative care models with old fashioned caring to better meet the needs of an aging population with chronic conditions. Responsibilities Clinical responsibilities, which may vary depending on physician expertise, interest and organizational needs, include in-home primary care, geriatric assessment and consulting, supervision of advanced practice nurses, and medical oversight of home care and hospice care. Opportunities available for leadership responsibilities, research activities, clinical program development, medical education, and integration of services in the Cleveland Clinic Health System and community facilities. Weekend call is shared and 1:6 and is mostly telephone/home call. Opportunities to split time with an office based practice, skilled nursing facility practice, virtual medicine role or in the inpatient care setting exist. Full time and part time opportunities available Flexible scheduling Qualifications Candidates must be board certified/eligible in Internal or Family Medicine; special interest and/or certification in Geriatric Medicine strongly desirable.
