Nurse Practitioner – Value‑Based Care | Contract | New Mexico (TX or NM Residents) Required Qualifications

• Master's degree or higher
• 2+ year of Telemedicine Care experience
• 3+ years of recent Primary Care experience
• 1+ year of longitudinal patient care experience
• 1+ year of experience working in a value‑based care model
• Full proficiency in speaking, reading, and writing Spanish- Preferred not required.
• Must live in Texas or New Mexico
• Active Nurse Practitioner license (NM or TX candidates able to obtain NM licensure)

This contract role offers an opportunity for an experienced Nurse Practitioner to help redefine care for medically complex patients in a growing, innovative value‑based care organization expanding into New Mexico . This is not a traditional clinic position—it's a chance to deliver proactive, home‑centered care through telehealth, remote monitoring, and interdisciplinary collaboration.

The ideal NP has strong clinical acumen, experience managing complex adult populations, and a passion for improving outcomes through a value‑based care model. Candidates with startup experience or comfort working in evolving care environments are highly encouraged to apply.

• Be part of a new market launch and help shape a modern care model for medically complex patients.
• Work within a collaborative interdisciplinary team including physicians, NPs, RNs, LCSWs, therapists, and care coordinators.
• Focus on prevention, early intervention, and improved outcomes , not high‑volume clinic throughput.
• Support patients where they need care most— at home , through telehealth and remote care management.
• Engage deeply with patients, families, caregivers, health plans, and primary care providers.

Patient populations may include:

• Medically complex adults
• Dual Eligible and Medicaid members
• Individuals with chronic conditions, disabilities, TBI, IDD, or functional limitations
• Medically fragile elderly populations

• Provide clinical care and ongoing management through telehealth and remote care technologies
• Manage medically complex Medicaid and Dual Eligible patient populations
• Develop and execute individualized care plans in collaboration with physicians and interdisciplinary teams
• Work closely with patients, families, caregivers, specialists, and health plan partners
• Identify changes in patient health and intervene early to prevent avoidable ED visits and hospitalizations
• Support patients through hospital admissions, discharges, and transitions back home
• Conduct advance care planning and goals‑of‑care discussions
• Participate in shared clinical triage and after‑hours support models
• Ensure accurate documentation supporting HCC coding, risk adjustment, STAR/HEDIS quality measures , and closure of care gaps
• Contribute to clinical best practices and help shape a growing care model

Pay Transparency Compensation varies based on location, experience, and performance. Some sites require disclosure of pay and benefits. This role may also be eligible for bonuses or incentives depending on business needs. Employees may receive health insurance, optional dental and vision coverage, life and disability insurance, retirement savings plans, paid holidays, and PTO or sick time. The estimated hourly rate for this position is $135k.
