A leading healthcare provider is seeking a dedicated Registered Nurse (RN) with strong home health and OASIS experience for a 13-week full-time contract assignment. Schedule:

• 13-week assignment, Monday through Friday
• Full-time hours; on-call responsibilities may be required
• Structured daily patient visit expectations

Key Responsibilities:

• Deliver skilled in-home nursing care and interventions
• Perform comprehensive patient assessments and complete accurate OASIS documentation within required timeframes
• Coordinate care and communicate closely with physicians and multidisciplinary teams to develop and implement individualized plans
• Complete wound care, medication administration, patient education, and chronic disease monitoring
• Provide timely communication of changes in patient condition to the clinical team

Qualifications:

• Active RN license
• At least one year of recent home health RN experience
• OASIS proficiency is required
• Strongly prefer candidates experienced with HomeCare HomeBase
• Comfortable with full-time, structured schedules and daily patient visits
• Reliable transportation and a valid drivers license
