As a Senior Living Rounding Physician with Twin Cities Physicians, you'll become a trusted medical presence for residents, caregivers, and families navigating complex healthcare situations. You’ll manage patients across assisted living, skilled nursing, memory care, and post-acute settings while helping families better understand conditions, treatment plans, and long-term care goals. Your expertise won’t simply impact diagnoses and medications, it will provide reassurance, guidance, and stability during some of life's most challenging moments. Responsibilities Provide on-site primary care and rounding services throughout assisted living, memory care, skilled nursing, and long-term care communities. Manage chronic disease, preventative care, transitional care, and post-acute medical needs. Perform diagnostics, treatment planning, medication management, and follow‑up care. Collaborate with Nurse Practitioners, Physician Assistants, nursing staff, therapists, and interdisciplinary teams. Support residents and families through complex medical decision‑making and care planning. Maintain accurate EMR documentation and coordinate ongoing care needs. Qualifications Active Minnesota Physician (MD or DO) license in good standing. Experience in geriatrics, internal medicine, family medicine, SNF care, post‑acute medicine, or related specialties preferred. Strong clinical judgment and confidence managing medically complex patient populations. Excellent communication skills and ability to build trust with residents and families. Valid driver’s license and reliable transportation.
