Community Health Connector II — Mentorship & Navigation

A community health organization in San Marcos is seeking an AmeriCorps Community Connector II to provide outreach, mentorship, and navigation services to individuals and families. This role involves conducting social needs screenings, mentoring Community Connector I members, and ensuring accurate documentation. Candidates should have at least a high school diploma, preferably some college or Community Health worker program experience, and strong communication skills.
