Onsite Care Navigator: Clinical Support & Community Linkage

CarePlus NJ, Inc. is seeking a Clinical Care Coordinator in Bergen (3 days) & Essex (2 days) Counties (Paramus & Bloomfield). This on-site, full-time role offers a 40-hour work week (Mon-Fri) with a starting hourly wage of $24.04 and a comprehensive benefits package. You will join a team dedicated to comprehensive care management, coordination, and connecting clients to community resources.
