Nurse Practitioner - In-Home Health Assessments

Overview Our client is seeking experienced Nurse Practitioners to perform in-home health assessments for Medicare and Medicaid members. This is an independent, field-based role requiring strong clinical judgment, excellent communication skills, and the ability to work in a wide variety of home environments. The role offers scheduling flexibility and requires availability of at least 10 hours per week. Key Responsibilities Perform in-home health assessments for Medicare Advantage members Work independently in a field-based setting Communicate effectively with members in a variety of home environments Apply strong clinical judgment during assessments Required Qualifications Active, unencumbered Nurse Practitioner license Minimum 6 months of Nurse Practitioner experience No new graduates Ability to work independently in the field Strong clinical judgment Excellent communication skills Availability for 10+ hours per week Preferred Qualifications Adult Gerontology Primary Care NP Adult NP Family NP Gerontology NP Acute Care NP, accepted only in specific states Certifications/Licensure Active NP license, unencumbered National Board Certification required: AANP or ANCC BLS/CPR required ACLS not accepted Mental Health board certification not accepted Pediatrics board certification not accepted Compensation Competitive pay How To Apply Apply through The Judge Group to be considered.
