Adult Gerontology Nurse Practitioner

Job Overview Urban Health Plan (UHP) is seeking Adult Geriatric Nurse Practitioners (AGNP) to deliver primary care services within our primary care teams across the Bronx, Queens, and Manhattan. The AGNP will work with a multidisciplinary team to care for patients, including performing examinations, conducting tests, analyzing records, diagnosing and preventing illnesses, and prescribing and administering medications, therapies, and vaccinations. Responsibilities Conduct patient examinations and perform medical tests. Analyze medical records, reports, and test results. Prevent and diagnose illnesses, diseases, or injuries. Prescribe, administer, and manage treatments, therapies, medications, and vaccinations. Explain procedures and test results, and develop treatment plans. Manage laboratory orders and referrals in coordination with the care team. Collaborate with physicians, PAs, NPs, and support staff as part of the clinical team. Perform additional duties as required by departmental needs. Qualifications Completion of an approved/accredited Nurse Practitioner educational program. Valid New York State Nurse Practitioner license. CDS, DEA, and BLS certifications. Strong communication skills and the ability to follow established procedures and protocols. Understanding of, and passion for, community health care (preferred). Benefits Federal/State loan repayment eligibility.
