DescriptionThe Transplant Coordinator is a Registered Professional Nurse, Nurse Practitioner or Physician Assistant with specific knowledge and skills who participates in the coordination of clinical programs and services for designated patient populations within the Transplantation Institute.ResponsibilitiesResponsible ForDevelopment, implementation, coordination and evaluation of clinical programs and services for designated patient populations.Principal Duties and ResponsibilitiesClinical PracticeCoordinates the multidisciplinary screening and evaluation of patients seen in the transplant practice at MSH and approved satellites. The multidisciplinary team includes the transplant surgeons, hepatologists, nephrologists, social work, nutrition, pharmacy and finance. Coordinators work with their assistants to facilitate other medical specialty visits as needed.Ensures all necessary lab tests are performed on potential transplant candidates and that results are updated as required (including bloodwork sent to Rogosin for tissue typing).Participates in making recommendations regarding placing as patient on the transplant waiting list. Educates patients on the benefits of living donation (when applicable).Communicates with all active transplant candidates regarding waiting list status on an ongoing basis. Ensures all medical clearances are updated regularly.On call responsibilities include facilitating all organ offers from UNOScommunicating with surgeons, fellows and patients to ensure timeliness of all organ offers. Coordinators also answer patient calls after hours, on holidays and weekends.Follows patients during their hospitalization and coordinates with transplant team to coordinate consulting services as needed. Coordinates outpatient follow up visits with the outpatient transplant team.Educates patients and family regarding all transplant medications, lifestyle changes, as well as outpatient follow-up protocols.Assesses continued recovery of patient through compliance with outpatient visits, scheduling of blood tests, regular review of labs, adjusts immunosuppressive medication as needed during regular lab review.Professional PracticeSupports the mission, philosophy, goals and objectives of the Department of Nursing and The Mount Sinai Medical Center.Maintains clinical competence.Is responsible and accountable for their own nursing practice.Advocates the highest standards of nursing practice.Approaches patients, families and other members of the health care team in a professional, respectful manner.Serves as a clinical role model and resource to the nursing staff.Serves as a professional resource to other members of the health care team.Demonstrates professional involvement by:Attending and participating in workshops, seminars, and courses as presenter or author.Keeping abreast of current literature and trends in practice.Participating in related professional organizations.Considers ethical issues in professional nursing practice.Treats co-workers, patients and families with dignity and respect.Maintains attendance and punctuality record in accordance with departmental standards.Adheres to dress code in accordance with departmental policy.Administrative PracticeCollaborates with other members of the health care team in the development, implementation, and ongoing review of policies, procedures, and standards of care for designated patient populations.Demonstrates knowledge of hospital, departmental and care center standards, policies, procedures and guidelines as well as regulatory agency standards.Participates in the development and implementation of continuous quality improvement programs.Participates in risk management activities including identifying and communicating issues of risk in a timely manner.Participates in committee activities.Is aware of and responsive to budgetary constraints and incorporates financial principles into program development and allocation of resources.Assists in the recruitment/retention, annual evaluations and disciplinary process for support staff of their designated organ program (i.e. transplant coordinator assistant and medical assistants).QualificationsEducation RequirementsBaccalaureate Degree in Nursing is required.Masters in Nursing and certification in specialty area are preferredOR:Baccalaureate Degree in Physician Assistant is required; Masters preferred.Experience RequirementsProgressive job related experience specific to area of responsibilityLicensing and Certification Requirements (if applicable)New York State Nurse/NP/PA License with current registration.Employer DescriptionStrength through Unity and InclusionThe Mount Sinai Health System is committed to fostering an environment where everyone can contribute to excellence. The salary range for the role is $107999.99 - $153000 Annually. Actual salaries depend on a variety of factors, including experience, education, and operational need.
