RN Case Manager - Hospice

(eligible after 1yr or 1000 hours worked)Our CultureLeadership is available to confer, consult and advise at any timeYou are encouraged to speak professionally and freelyWork-life balance is a top priorityLaughter, passion for personable-care, and a supportive environmentRN ResponsibilitiesComplete ongoing assessments of the patient’s physical status and the psycho-social status of the patient/familyInitiate and develop a plan of care in collaboration with the Interdisciplinary Group (IDG), the patient/family, and the attending physicianProvides education and interventions to support the effective management of the patient’s pain and other physical symptoms as well as assesses the patient/family’s response to intervention and treatmentEnsures that equipment, supplies, drugs and biologicals, and other necessary items and services are availableMake necessary revisions to the IDG plan of care as the patient’s/family’s status and needs changeCompletes accurate and timely documentation of patient/family services providedRegularly attends IDG meetingsRN RequirementsRN with active, unrestricted license in the State of MichiganAt least one (1) year of relevant experience as an RN (required)One (1) year of management experience (preferred)One (1) year of home health experience (preferred)Current CPR CertificationCommunity health and a wide range of Medical-Surgical experience (preferred)Reliable transportation with a valid state driver's license and current car insuranceInterim HealthCare is an Equal Opportunity Employer. #J-18808-Ljbffr
