What You'll Do

• Build and strengthen relationships with physicians, hospitals, case managers, skilled nursing facilities, assisted living communities, and other referral partners
• Develop new referral sources while maintaining and growing existing accounts
• Represent Choice Health at Home as the preferred provider for exceptional patient care
• Meet and exceed monthly admission and census growth goals
• Partner closely with clinical teams to ensure smooth patient transitions and quality outcomes
• Execute strategic territory planning and community outreach initiatives
• Maintain compliance with all healthcare marketing regulations and agency standards

What We're Looking For

• Recent home health or hospice marketing/sales experience required
• Proven success growing referrals and building strong healthcare relationships
• Existing physician and referral source relationships preferred
• Self-motivated, goal-oriented, and relationship-driven professional
• Excellent communication and presentation skills
• CRM and HomeCare HomeBase experience preferred
• Valid driver's license and auto insurance required
