Position Summary

The RN/Case Manager is a member of the hospice interdisciplinary group (IDG) and works collaboratively with physicians, nursing staff, hospice aides, social workers, chaplains, and other members of the care team to address the physical, medical, psychosocial, emotional, and spiritual needs of patients and their families.

The RN/Case Manager coordinates and provides nursing care in accordance with the patient's individualized Plan of Care, performs ongoing assessments, communicates changes in condition, provides patient and caregiver education, and helps ensure services remain responsive to the patient's changing needs.

Scope of Work

Nursing & Case Management

• Plan, coordinate, and provide hospice nursing care in accordance with the patient's individualized Plan of Care.

• Perform ongoing nursing assessments and evaluate changes in the patient's condition, symptoms, functional status, and care needs.

• Utilize the nursing process, including assessment, planning, intervention, implementation, and evaluation.

• Coordinate patient care with members of the interdisciplinary team, patients, families, caregivers, and other healthcare providers.

• Communicate significant changes in patient condition to the appropriate physician, Hospice Medical Director, and interdisciplinary team.

• Obtain and implement medical orders in accordance with applicable law, professional scope of practice, and organizational policy.

• Assist with symptom management, comfort-focused care, medication management, and other nursing interventions appropriate to hospice care.

• Maintain continuity of care throughout the patient's hospice experience.

Plan of Care

• Participate in the development, review, implementation, and ongoing update of the patient's individualized Plan of Care.

• Identify changes in patient or caregiver needs and coordinate appropriate updates to interventions and services.

• Ensure the Plan of Care reflects current clinical needs, goals, physician orders, and interdisciplinary recommendations.

• Participate in interdisciplinary group meetings, case conferences, and other care-planning activities.

Patient & Family Education

• Educate patients, families, and caregivers regarding the hospice philosophy, disease progression, symptom management, medications, safety, the dying process, and the patient's Plan of Care.

• Provide guidance and support to patients and caregivers regarding healthcare decisions within the scope of the RN role.

• Reinforce education throughout the course of care as patient and family needs change.

• Provide education and clinical guidance to referral sources and other care partners as appropriate.

Care Coordination

• Coordinate services and referrals necessary to support the patient's Plan of Care.

• Assist with coordination of services such as physical therapy, occupational therapy, pharmacy services, medical supplies, durable medical equipment, and other appropriate consultative services.

• Collaborate with facility staff, physicians, pharmacies, DME providers, and other healthcare partners involved in the patient's care.

• Provide appropriate written and verbal handoff communication to promote continuity and safety of care.

Clinical Supervision & Team Support

• Provide nursing direction, education, and oversight to LPNs and hospice aides/STNAs as appropriate to assigned responsibilities, scope of practice, the patient's Plan of Care, and organizational policy.

• Communicate patient-specific instructions and changes in care needs to appropriate members of the nursing and interdisciplinary teams.

• Support clinical staff in maintaining safe, consistent, and patient-centered care.

Documentation

• Complete accurate, timely, relevant, and thorough clinical documentation.

• Document nursing assessments, interventions, symptom management, patient and caregiver education, physician communication, care coordination, changes in condition, and patient responses to care.

• Ensure clinical documentation and care plans remain current and consistent with services being provided.

• Maintain documentation in accordance with organizational policy and applicable regulatory requirements.

Interdisciplinary Collaboration

• Participate in regularly scheduled interdisciplinary group meetings and case conferences.

• Collaborate with the Hospice Medical Director, attending physicians, social workers, chaplains, hospice aides, LPNs, therapists, and other members of the care team.

• Communicate changes in patient condition, concerns, unmet needs, and care recommendations to the appropriate team members.

On-Call Responsibilities

• Participate in the organization's on-call rotation based on program and patient-care needs.

• Respond appropriately to patient and caregiver concerns during assigned on-call periods.

• Provide nursing assessment, guidance, intervention, and escalation as clinically appropriate.

Technology

• Utilize the Electronic Health Record (EHR), email, computers, mobile devices, and other technology required for clinical documentation and communication.

Qualifications

• Minimum of one year of professional Registered Nurse experience.

• Hospice, home health, palliative care, long-term care, or related clinical experience preferred.

• Strong nursing assessment and critical-thinking skills.

• Knowledge of nursing case-management principles and care coordination.

• Strong organizational and time-management skills.

• Ability to prioritize changing patient and caregiver needs.

• Strong communication and interpersonal skills.

• Ability to communicate effectively both verbally and in writing.

• Ability to work independently in community-based settings while collaborating effectively with an interdisciplinary team.

• Valid driver's license and access to a reliable, insured automobile.

Education & Licensure

• Graduate of an accredited School of Nursing.

• Current, active, and unencumbered Registered Nurse (RN) license in the State of Ohio.

• Must maintain all professional credentials and qualifications required for the position.

Job Conditions & Physical Requirements

• Position requires travel throughout Marion, Delaware, and surrounding service areas.

• Must be able to travel to patients' homes, facilities, hospitals, and other care settings.

• Must be able to access patient residences and care environments that may have limited accessibility.

• Physical activities may include walking, sitting, standing, bending, stooping, and other activities associated with providing nursing care.

• Must be able to lift up to 50 pounds.

• Must be able to perform the essential functions of the position, with or without reasonable accommodation.

• Participation in the organization's on-call rotation is required.
