Home Health Care, Inc. seeks an RN Admissions Nurse to lead Start of Care admissions across the Twin Cities Metro. You’ll perform comprehensive nursing assessments, complete OASIS documentation, and create individualized care plans that set patients up for success. This role offers independence with a territory-based schedule, education on diagnoses and services for patients and families, coordination with physicians and the care team, and strong support from experienced clinical managers. #J-18808-Ljbffr Mytcp
