About This Role The Care Manager develops, assesses, and facilitates complex care management activities primarily for members with mental and behavioral health needs, aiming to provide high-quality, cost-effective healthcare outcomes. Highlights Pay: $56,200 - $101,000/year Location: Remote, FL Schedule: Full Time, Monday-Friday, 8am - 5pm ET Residency: Must reside in Florida What You'll Do Evaluate member needs via phone or in-home visits and recommend/facilitate care plans. Perform telephonic, digital, home, and/or other site visits outreach to assess member needs. Develop ongoing care plans for members with high acuity and identify necessary providers and resources. Coordinate between members, families, community resources, and care provider teams. Monitor care plans and member status for changes in treatment and provide recommendations. Facilitate care coordination and collaborate with providers to ensure timely access to services. Collect, document, and maintain member information to ensure compliance with regulations. Provide education to members and families on procedures, treatment options, and healthcare benefits. Provide feedback to leadership on opportunities to improve care delivery. Perform other duties as assigned and comply with all policies and standards.
